Collaborative Care for Older Adults with Cognitive and Behavioural Concerns RESPOND Care partners with General Practitioners to support the assessment and management of older patients experiencing cognitive decline, behavioural changes, or complex care needs.
As a Nurse Practitioner-led outreach service, we provide:
Targeted behavioural and cognitive assessments
Individualised approach and recommendations tailored to the client
Support across residential aged care and community settings
Timely communication and collaborative decision-making
